Alive certificate for a Bangladeshi pension: free, in person or with a notarized affidavit

Alive certificate
A Bangladeshi government pensioner living in the U.S. needs an alive certificate to keep the pension paid. The Embassy and Los Angeles issue it free of charge, in person or, if you cannot come, with a notarized affidavit that you are alive.

Checked against the official pages on 22 October 2026. How we check.

The Embassy says the alive certificate pensioners need is issued free, against a notarized affidavit that the pensioner is alive (Embassy page). Los Angeles describes it for retired government employees or their spouses: it states that you were alive on the day it was issued (Los Angeles page).

In person or by mail

At Los Angeles the certificate is issued when you come in person with your Bangladeshi passport and earlier certificates.

The alive certificate, two ways

If you cannot come, you apply by mail with the application form, an affidavit notarized and authenticated by the city or county clerk, a copy of the first five pages of your passport and a prepaid return envelope (Los Angeles page).

What Embassy Los Angeles
Fee Free Free
In person Yes Yes, with your passport
Without coming Notarized affidavit By mail, affidavit authenticated by the clerk

Los Angeles keeps the mail route for seniors who cannot come and live outside the city (its page says “outside the five boroughs of Los Angeles”).

The papers go to the Consulate General of Bangladesh, 501 South Fairfax Ave, Los Angeles, CA 90036, with a self-addressed prepaid envelope for the return (Los Angeles page).

New York’s own website lists the alive certificate among its services too; its page gives no fee (bdcgny.org).

New York’s own website asks the retired government servant to come in person with a copy of the Bangladeshi passport, the government order of retirement and the pension book, or the alive certificate New York issued last time (bdcgny.org).

Office What to bring Without coming
Embassy Form-C and a notarized affidavit that you are alive The notarized affidavit
Los Angeles Bangladeshi passport and earlier certificates Form, affidavit authenticated by the clerk, passport copy
New York Passport copy, retirement order, pension book or last certificate Not described

The rows come from each office’s page: the Embassy issues the certificate on its attestation form, Form-C (Embassy page), Los Angeles describes the mail route (Los Angeles page) and New York lists the papers for a visit (bdcgny.org).

Transfer of residence

The same Embassy page describes a certificate on transfer of residence for someone who has lived in the U.S. for at least two years in a row and is moving back to Bangladesh, with proof such as employment records or a lease (Embassy page).

Your passport has expired

Bring it anyway with your other papers; to renew it, see Bangladeshi passport.

A spouse’s pension

Los Angeles mentions spouses of retired government employees among the people who can apply (Los Angeles page).
